FREE and CONFIDENTIAL  emotional wellness screens for youth ages 3 - 21 in Chautauqua County, New York

Welcome to Chautauqua County Early Identification & Recognition, a grant program sponsored by the NYS Office of Mental Health. The host of the grant is Chautauqua County Department of Mental Hygiene. By partnering with primary care, pediatricians, schools, and other child-servicing agencies, we promote early emotional wellness screens for all youth ages 3-21. The screening tool used is a non-diagnostic short questionnaire that is filled out by a parent or caregiver and sometimes the young person themselves; similar to a vision or hearing screen. We know that taking care of our mental health is just as important as our physical health.
